Provider Applications Are Available

Applications are now available for
Agency-Based Providers and Hospital/Psychiatric Units to complete in hard copy form. Partners BHM is currently working to develop
an electronic version of these applications for providers to complete and
submit. Providers may choose to wait to
complete an electronic application along with supporting documentation once it
becomes available in the very near future. To access the applications through
the website, go to http://www.partnersbhm.org/providers/provider-forms.aspx.
Applications
for Licensed Independent Practitioners are not yet available, but will be made
available on the website as soon as Partners BHM has secured a contractual
agreement with a Credentialing Verification Organization (CVO). Licensed or
Certified Practitioners that need to complete the process include: MD, DO,
Practicing Psychologist (PhD, PsyD), Licensed Psychological Associate, LCSW,
LMFT, LPC, LCAS, Certified Substance Abuse Professional, Advanced Practice
Psychiatric Clinical Nurse Specialist, Psychiatric Nurse Practitioner and
Physician Assistant.
All providers are required to complete and return
enrollment to Partners BHM no later than close of business on October 3, 2012 (90 days prior to “go
live” date) to ensure the completed application will be processed by January 1, 2013. Applications
received after October 3, 2012 will
be processed in the order received; however, there is no assurance the
application will be processed by January
1, 2013. In this case, the provider
must develop a transition plan to transition consumers in the event the
provider cannot be enrolled with Partners BHM network by January 1, 2013.
A minimum of 90 days is required to complete the credentialing
process including verification of “good standing”, contracting and
enrollment. As of January 1, 2013, Partners BHM will only add providers to the
network based on identified consumer need.
Providers
are reminded that to enroll with Partners BHM that they must be in “Good
Standing” and have billed a service to a consumer within the eight county
catchment area within sixty (60) days prior to enrollment.
