Each year, Partners surveys individuals in our coverage area to determine how we can improve the availability and accessibility of services for people with mental illness, substance use disorders and intellectual and developmental disabilities. The Community Needs Assessment Survey is one way we gather individual perspectives of the health care needs in our communities.

How Does the Survey Results Affect My Community?
Each year’s survey results are used to develop our annual Community Network Adequacy and Accessibility Analysis. This report identifies gaps in health care services and community resources, and contains data that helps Partners develop positive solutions that impact the health of our community.
Some of the services developed from previous survey results include:
- Additional residential settings for youth with substance use disorders. More office-based treatment providers.
- The opening of six Oxford Houses across the Partners’ region.
- New youth services, like High-Fidelity Wraparound, Mobile Outreach Response and Stabilization (MORES), Young Adults in Transition, and Long-Term Community Supports.
- Partners’ MobileCONNECT, a platform that helps members access telehealth services and monitor their health.
