2026 Perceptions of Care Survey

The 2026 Perceptions of Care Survey administration period is Aug. 10-Sept. 18, 2026. Partners is asking all mental health and substance use providers to administer as many surveys as possible to members who received services from July 2025 through June 2026, including IIH, residential, ACTT and SUD services.

Members who received enhanced services are preferred, but providers may survey other eligible members.

Participation Goals

Partners’ goals are to receive:

• 700 completed adult surveys
• 200 completed youth surveys
• 125 completed child and family surveys

Administration Options

Providers may administer surveys using web-based, paper, telephone or two-way video methods. English and Spanish versions are available for adult, youth and child and family populations.

All responses must be submitted electronically through the appropriate survey link by Sept. 18. If a member completes a paper survey, the provider must enter the responses online and then shred the paper copy. Do not mail completed surveys to Partners.

Survey Requirements

To ensure a survey is counted, providers must:

• Complete all provider questions before the member begins the survey.
• Enter the provider location’s 10-digit NPI, not an individual clinician or corporate NPI.
• Enter the member’s Alpha number, not the provider’s internal identification number.
• Enter the member’s Medicaid number.
• Access the survey provider page each time a new survey is started. Using the browser’s back button may overwrite the previous survey.
• Select “Submit” at the end of the survey. Selecting “Next” does not save responses.

The survey provider page and individual survey links are for authorized agency staff only. Do not send or share them with members or anyone outside your agency.
