Welcome to the Partners Health Management County Resources page! We’re here to help with mental health and substance use challenges. Below, you’ll find info about your local health department, county-specific grants, community collaboratives and more. Our Community Collaboratives bring together different groups like families, service providers, healthcare professionals and advocates. We work together to solve problems and support youth, adults and families in our communities. Click on your county or explore below for info and services.
