Integrated Care Centers

At Burke Integrated Health, local providers and Partners have joined forces to offer integrated care to people in Burke County by providing primary health care and specialty behavioral health care in one location.

Health care providers evaluate every level of the person’s wellbeing – medical, behavioral, and social determinents. Then, whole-person care is coordinated and integrated through a team-based approach to address primary care, medical problems, mental illness, substance use disorder, and social issues.

In December 2015, the NC Council of Community Programs awarded Partners and Burke Integrated Health with the Programs of Excellence Award in Care Integration for successfully developing a location where citizens can receive primary healthcare, mental healthcare, and substance use treatment.

Impact Health of Iredell is a project between local providers and Partners allowing Iredell County citizens to have primary care and behavioral health needs treated at one place by a team of professionals.

The center approaches an individual’s care using Partners’ Whole Person Integrated Care model. When someone comes to Impact Health of Iredell for help, providers will address the person’s primary care, behavioral health, and social determinants of health such as emotional, financial, spiritual, occupational wellness.

Impact Health of Iredell connects individuals and families with the health care and social services and resources they need. Services are available for adults and children of all backgrounds, including disabilities and special needs.

The Lincoln Wellness Center is a collaborative effort to provide behavioral health services and crisis response to Lincoln County citizens.

Through referrals, participation in events and meetings, and cooperative services, the center works to divert psychiatric emergencies to more appropriate resources, initiate services for new recipients, and increase opportunities for follow up and treatment established recipients of mental health and substance use treatment.

In December 2015, the NC Council of Community Programs awarded Partners and Lincoln Wellness Center with the Programs of Excellence Award for Crisis Response for developing new and more effective crisis response services and protocols.

The Ollie Harris Center is a formal collaboration between providers and Partners to provide greater access to a wide variety of treatment and resources in Cleveland County.

People desperately in need of mental health or substance use disorder services can reach professionals and start care faster than ever before, due to a unique urgent access and shared triage process. The center’s staff coordinates services and access to treatment between the person being referred, community organizations, and providers at the center.

Shared triage is a process of gathering and sharing information to improve access to care and timely treatment. Everyone coming to the center for the first time starts with a referral coordinator. Based on answers to the core questions about signs and symptoms combined with information from referring professionals, the coordinator can schedule a comprehensive clinical assessment with the most appropriate provider, right away. Sometimes assessments can be done within an hour or two.
