If you do not have a primary care provider (PCP), you can use the Provider Search Tool to find one. You can change your PCP at any time.
Primary Care Provider (PCP)
You can choose or change your PCP by:
- Submitting the online PCP Change Request form
- Downloading and completing the PCP Change Request form (English) (Español)
Tailored Care Management (TCM)
You can use the TCM Request Form to:
- Select a Tailored Care Management provider (opt in)
- Opt out of Tailored Care Management
Complete the TCM form online.
