Confidential Online Mental Health and Wellness Tools

We all have times when we struggle. How often you have these feeling and how long they last are different for each of us, but they can mean we need to get help from a doctor or mental health professional. Finding support to focus on your emotional health is important. Partners wants you to have the tools you need to get better and stay mentally strong.

We have several online tools to help you decide if you might need help or have a mental health or substance use issue. These tools are made to find out how you’re feeling and help you decide the next steps to take. All tools keep your personal health and private information safe. This means we ensure that your electronic protected health information (e-PHI) is secure and is not available or disclosed to unauthorized people.

HealthCheck 20/20

HealthCheck 20/20 helps identify co-existing behavioral health conditions. It supports earlier identification and care for mental health and substance use disorders.

Self-Help Assessments

Self-Help Assessments help you see areas in your life where you might need help. Self-help tools help you better manage your thoughts, feelings and behaviors. MemberCONNECT

Partners Tailored Plan has joined with Pyx Health to offer you a phone app that focuses on daily mood and wellness check-ins. Pyx Health gives you access to resources and tips to live a healthier life. Pyx Health also offers mental health screenings, positive text messaging and assistance, if needed, either through the app or by calling the Pyx Health Compassionate Call Center.

Follow the steps below to get started:

Partners Training Resource and Collective (PartnersTRAC)

Partners Health Management makes it easy for members to access training, articles and tools to help you live a healthier life. We offer webinars, in-person events, videos, recorded presentations, and written information.

Search PartnersTRAC for articles and information on Tobacco Cessation, Opioid Use, Diabetes Prevention and Management, Stress Management, Healthy Eating, The Benefits of Exercise and more.

Do you need a list of transportation agencies near you or Veterans’ resources in your community? We have a list of agencies and resources that supply the programs and support you need to help you improve your health and wellness.

Finally, PartnersTRAC program offers trainings and educational events, usually at no cost to you, on topics to benefit the health and wellness of our members.

Healthy Opportunities

If you live in or your Medicaid services comes from Burke or Rutherford counties, and you have at least one physical/behavioral health risk factor and at least one social risk factor, you may be eligible to receive additional services and supports free of charge for non-medical needs related to securing stable housing, accessing nutritious foods, receiving reliable transportation, and addressing interpersonal violence and toxic stress. Contact your care manager or call Member and Recipient Services at 1-888-235-4673 to see if you qualify and learn what services are available to you.

Physical/Behavioral Health Risk Factors

  • Adults (such as two or more chronic conditions)
  • Pregnant women (such as multifetal gestation)
  • Children, age 0-3 (such as a baby that was in a neonatal intensive care unit)
  • Children, age 0-21 (such as experiencing three or more adverse childhood experiences)

Social Risk Factors:

  • Homeless and/or housing insecure
  • Food insecure
  • Lack of transportation
  • At risk of, witnessing or experiencing interpersonal violence
Other Programs to Help You Stay Healthy

Partners Tailored Plan wants to help you and your family get and stay healthy. If you want to quit using tobacco or are a new parent who wants to learn more about how to best feed your baby, we can connect you with the right program for support.

Opioid Misuse Prevention Program – Project Reducing Harm from Opioids (RHO)

Opioids are powerful prescription medications that can be the right choice for treating severe pain. However, opioids may also have serious side effects, such as addiction and overdose. Partners Tailored Plan supports safe and proper opioid use through our Opioid Misuse Prevention Program. Health Support Services

At Partners, we understand that there are many things that can affect a person’s health. That is why we have a team of dedicated member engagement specialists and care managers to help you.

Member Engagement Specialists

Our member engagement specialists will help you:

  • Learn your rights and responsibilities.
  • Create a Psychiatric Advance Directive (PAD).
  • Find community resources and supports.
  • Understand the Medicaid system.
  • Access handbooks and member materials.

Care Managers

Managing your health care alone can be hard, especially if you are dealing with many health problems at the same time. If you need extra support to get and stay healthy, we can help. As a member of Partners Tailored Plan, you can have a care manager on your health care team for services that do not already include care management.

A care manager is a specially trained health care worker who works with you and all your health care providers, including your doctors, to make sure you get the right care when and where you need it. The care manager knows what resources are available in your community and will work with local providers to get you the help you need.
