There are times when you may need to submit a Prior Authorization Request and ProviderCONNECT is not operational.

During those times only, providers may use the Manual Prior Authorization Request form.

Please note that new benefit plans effective April 1, 2023 for Tailored Plan/PIHP/State funds will be available for view March 1, 2023.
