The Partners’ Tailored Plan Utilization Management (UM) program has an integrated system of care for its members. The program looks at a member’s need for:

• Behavioral health services
• Crisis services
• Physical health services
• Long-term supports and services
• Traumatic Brain Injury services
• Intellectual and Developmental Disability Services
• Pharmacy services
• Other unmet service needs

Our program brings together the knowledge of Partners, Carolina Complete Health and CVS Caremark to create this approach. Partners works with our health care teams to meet the member’s whole health needs.
