Your Partners Tailored Plan gives you the care and support you need to stay healthy.
Partners Tailored Plan offers State-Funded Services for people who do not have Medicaid, are uninsured or are underinsured. These services cover mental health, substance use, intellectual or developmental disabilities (I/DD) and traumatic brain injury (TBI) needs. Services are based on available funding and may be limited. You must use providers in the Partners network.
