Davidson County Realignment Information

Individuals in Davidson County currently receiving Medicaid for intellectual or developmental disabilities (I/DD), mental health or substance use disorders through Sandhills will be automatically enrolled with Partners Health Management on Feb. 1, 2024.

Partners is a Local Management Entity/Managed Care Organization (LME/MCO) responsible for ensuring access to services for people who:

  • Are receiving Innovations services.
  • Are on the I/DD waitlist.
  • Have significant behavioral health needs.

Partners manages a system of care that is shaped by the choices of our members and their families, and is designed to meet the unique needs of each community in our service area.

Davidson County Provider Information Sessions

Please join Partners Health Management for a Provider Information Session regarding County Realignment and welcoming Davidson County Providers. If you are serving Davidson County Members, we invite you to join us about Provider Contracting. We will hold two sessions in December: December 5 and December 12, 2023.

Please register in advance for one of the two sessions. After registering for either date, you will receive a confirmation email containing information about joining the meeting.

Why is this changing?

North Carolina Department of Health and Human Services (NCDHHS) Kody Kinsley released his plan to consolidate the LME/MCOs in November, 2023. Under the Secretary’s plan, Sandhills Center will be dissolved with all but three Sandhills counties joining Eastpointe. The plan also calls for Eastpointe and Trillium Health Resources to consolidate, resulting in four LME/MCOs with a minimum population of 1.5 million each.

The following county transitions will be effective February 1, 2024:

  • Davidson County will be realigned to Partners.
  • Harnett County will be realigned to Alliance Health.
  • Rockingham County will be realigned to Vaya Health.
  • All other Sandhills counties will join Eastpointe.

How does this affect me?

  • Beginning February 1, 2024, residents in Davidson County who receive Medicaid services from Sandhills Center will automatically begin receiving services from Partners.
  • If you transitioned to a Standard Plan on July 1, 2021, you will not be affected by this change and will continue to receive your services from your selected standard plan.

Am I eligible for State-funding?

State Eligibility Income Requirements/Financial Eligibility

To qualify for State funding, the household income for the recipient must be 300% or at least three times lower than the most recent federal poverty guidelines, based on family size. If a recipient’s household income exceeds this amount, (household size and poverty guideline amount times three), the recipient is not eligible for State funding. The most current version of the Federal Poverty Guidelines can be found here.  Income guidelines do not apply and are waived for crisis/emergent events, traumatic brain injury (TBI) or intellectual/developmental disabilities I/DD).

Information for Providers

Providers must submit a completed enrollment form to Partners now to ensure a seamless transition of services for daily service/Assertive Community Treatment Team (ACTT) recipients receiving a State-funded services.  

What do providers need to do?

Providers need to submit enrollment forms to Partners for individuals served with State-funding. There are different methods for enrollment depending on the type of service received.

Recipients receiving Daily Service/ACTT through State-funding: (This includes individuals with State-funding only and individuals with active Medicaid Direct that receive an enhanced daily service that requires a State-funding layer.)

County Transition – Recipient Enrollment for State-Funded Form – Daily/ACTT Services >

  • Complete the entire Recipient Enrollment for State-Funded Form for Daily Services/ ACTT. Ensure that the admission date, diagnosis and benefit plan effective dates are all Feb. 1, 2024, and use DSM-V admission codes.
  • Fax the form to 336-835-2075, and include “Partners, ATTN: County Transition” in the subject line OR send via secured email to EligibilityEnrollment@PartnersBHM.orgSecure email is the preferred method for submission.
  • The deadline for providers to submit this information is 5 p.m. on Tuesday, Jan. 30, 2024. After this date, submit enrollments through Alpha+ starting Feb. 1, 2024.

Individuals receiving State-funded Outpatient Services:

  • During the individual’s next scheduled office visit on or after Feb. 1, 2024, log in to Alpha+. Note that Alpha+ will not permit you to enter a date in the future for admission.
  • Complete an Enrollment Form within Alpha+. Update demographic and financial information. Providers can access Alpha+ here.
  • Note: Partners normally requires enrollments within seven calendar days from the date of admission, or the next appointment with the State-funded recipient. During the transition, providers serving individuals from Davidson County will be given an extended period to enter enrollments. Partners is allowing a 30-day grace period. Effective March 3, 2024, we will revert to our policy requiring enrollments within seven calendar days from the date of admission.

Mobile Crisis Management/Facility Based Crisis:

Individuals who engage in Mobile Crisis Management (MCM) or Facility Based Crisis (FBC) services on or after Feb. 1, 2024, should be enrolled in Alpha+.

Eligibility Documentation

To ensure accuracy of information and eligibility, providers are requested to attach/upload Eligibility Documentation to recipient documents in Alpha+ when documents are available and provided by the recipient. Documents to be uploaded are:

  1. State issued driver’s license/identification card (in the event of a minor, the parent or legal guardian’s driver license/identification card is required).
  2. Social security card.
  3. Income verification in the form of a recipient’s paystub, tax return, or in the event of no income, a signed and dated Statement of No Income Verification Form.

Documents should be attached/uploaded upon initial Enrollment and upon each annual Client Update Request to add a new annual benefit plan.

Providers may also upload these documents to the recipient’s record, after an enrollment has been approved and a record has been created, through the Patient Maintenance module under Clinical Documents > Eligibility Documentation. If a recipient has an existing record in Alpha+, you do not need to wait until enrollment has been approved to attach documentation.

Please be advised that, if documents are unavailable, the recipient will not be denied services.
